Sunrise at Five Forks is an assisted living and memory care community on Five Forks Trickum Road in Lilburn, Gwinnett County, in a Victorian-style building with walking trails and gardens. It also takes short-term stays, for recovery after surgery or a trial run. Care is priced on top of the apartment: a nurse sets a care level before move-in, and medication management is billed separately again.
